V-034 SPONTANEOUS EVISCERATION THROUGH AN UMBILICAL HERNIA

Abstract Aim Umbilical hernia in the cirrhotic patient is frequently seen setting of refractory ascites. Nevertheless, spontaneous evisceration an unusual and potentially life-threating event. This rupture has a high mortality morbidity presents many challenges medical surgical management. Material & Methods We present case 58 years-old man with personal history alcoholic cirrhosis multiple ascites drainages, portal hypertension hepatic encephalopathy. The went to emergency department suffering from umbilical evisceration. Results An urgent hernioplasty was conducted. performed sac suprafascial dissection. verified that vascularization content correct. close peritoneum double suture (transfixion ligature continuous techniques) fascia defect. finally placed onlay mesh. In postoperative period, had no complications related dehiscence or peritoneal fluid leakage, but stayed hospitalized for 25 days due decompensation cirrhosis. After six months follow-up, there are signs recurrence. Conclusions Cirrhotic patients undergoing abdominal surgery prone complications, especially hernias One de most frightening leakage fluid. our case, this could not have been produced because we used transfixion ligation peritoneum.
